This is a copy for review before purchase.  If you agree  Please print the contract, fill in your name, attach the deposit check of $500.00 or pay by PayPal, and sign and date this document.  Then mail the signed contract to :

One week before the puppy will be ready to be picked up or shipped, the entire purchase amount must be paid.   Your copy of this contract and all registration and microchip information will be sent to you by registered mail at the time the puppy is shipped.

             Contract and  Health Guarantee

On this __________day of _______________ 2008, this Male/Female puppy is being sold to ________________________________________for the price of $_________________.  (Shipping, if necessary, is additional).  The $500.00 deposit  accompanies this contract. 

This puppy has had ________series of puppy shots, and has been wormed.   This puppy has been implanted with a microchip that is to be recorded with your Vet. or other organization to help locate your puppy if lost.  Chip #_______________________________

 The vet has checked this puppy and it appears to be in perfect health.

Therefore: 

This puppy is guaranteed for it's first year of life, against LIFE THREATENING CONGENITAL ANOMALIES,  such as  LIVER SHUNT,  OR A DEFECTIVE HEART.  If this puppy develops such a congenital anomalies within the 1st year of life, I will replace the puppy with a male/female puppy of like value.  I will not be responsible for medical care to treat or cure such anomalies.  If you decide to keep the puppy, I will consider a partial refund decided on a case - by - case basis. 

The Vet that finds, treats, or cures said anomalies will be required to write a detailed description of the anomalies and the treatment plan and prognosis on the Vet's official letterhead, and be available for discussion of the plan and prognosis with my vet.   In the event the puppy dies, the puppy must be autopsied by a certified vet with tissue samples, etc. sent in for analysis stating the cause of death.

This certifies that the new owner has been warned about the possibilities of hypoglycemia (low blood sugar).  Death or injury caused by hypoglycemia will be the sole responsibility of the new owner.

This certifies that injuries  that occur after ownership is transferred, (the puppy is dropped, hit, kicked, stepped on, sat on, choked, or mauled by another animal, which results in any physical problem such as seizures, injured joints, death, etc.) are the sole responsibilities of the new owner .

This certifies that this guarantee is for the original owner only and is not transferable if this puppy is resold.

This certifies that if you are no longer able or willing to keep your baby, we will get first opportunity to repurchase the puppy, and if you sell your puppy to another loving family, you will send new owner information to this kennel for our records.
